Parents can Opt Out of Answering Gender Specific UIL Student Medical History Form Questions by School Year 2024-25

The UIL Medical Advisory Committee voted unanimously (April 16) to allow parents to be able to check a box on this preparticipation medical history form to opt themselves out of having to answer gender specific medically related questions.
